Overview:

Rosewell Adult Day Center provides a safe, structured, and engaging environment for adults
who require daytime supervision, assistance, and support. The program is designed to
enhance the quality of life for aging adults, individuals with mild cognitive impairment to mild to moderate dementia, and adults with intellectual or developmental disabilities and chronic health
conditions. Our mission is to promote independence, dignity, and meaningful social
engagement through person-centered care and evidence-based practices.

Program Purpose:
The primary purpose of Rosewell Adult Day Center is to offer comprehensive day center
services that foster physical, emotional, cognitive, and social well-being. We aim to relieve
caregiver stress while allowing members to maintain their independence and remain in
their homes and communities as long as possible.

Philosophy:
Rosewell Adult Day Center believes that every individual deserves compassionate,
respectful, and empowering care. Through person-centered planning, continuous staff
education, and collaboration with families and community resources, we promote holistic
wellness for every member entrusted to our care.

Program Environment:

The center features a warm, home-like atmosphere with designated areas for group
activities, dining, rest, and therapeutic programming. Safety and accessibility are prioritized through ADA-compliant design, secure entry systems, and trained staff available at all times.

Admission criteria & Pre-admission requirements:

To be eligible for admission, the following criteria must be met:

• All participants shall be 18 years of age or older
• All participants shall be able to self-preserve themselves in an emergency

• The individual requires supervision, personal care, assistance with activities of daily living, and/or structured programming.
• The individual’s needs can be safely met by the center’s staffing, services, and environment.
• The individual does not require continuous skilled nursing or acute medical care.
• The individual does not pose a risk to self or others that cannot be safely managed.
• The individual can participate in a group setting with reasonable accommodations.
• The center has sufficient staffing, space, and resources to meet the participant’s needs.

Medication needs
Dietary needs and restrictions
Mobility and evacuation capability
Psychosocial needs

Standard program rate is:

Rate:
• $85.00 per day for standard adult day services
• Additional services (e.g., medication administration, enhanced personal care, special programming) may result in additional charges and will be disclosed in advance.
Frequency of Payments
Payment for services is due weekly, in advance or no later than the first scheduled day of attendance for each service week, unless alternative arrangements are approved in writing by the Center.
Advance or Deposit Payments
Any advance payment or deposit required shall be identified, including the amount, purpose, and the Center’s refund policy for such payment.
Accepted payment methods and billing procedures shall be provided to the participant or legal representative at admission.

Service offered:

• Individual & Group Services: Assistance with life skills, adaptive behaviors, and social integration activities tailored to individual goals.
• Health & Wellness Monitoring: Supervision of daily health status, medication reminders, and coordination with healthcare providers as needed.
• Therapeutic & Recreational Activities: Structured programs including exercise, art therapy, music therapy, memory games, sensory stimulation, and community outings.
• Nutrition Services: Nutritious meals and snacks are provided daily, with accommodations for dietary restrictions and physician-ordered diets.
• Personal Care Assistance: Support with activities of daily living (ADLs) such as grooming, toileting, and mobility assistance.
• Caregiver Support & Education: Resources, counseling referrals, and respite for family caregivers.
